Note: Sitecore Forms module has been replaced on Sitecore CMS 6 with the Web Forms for Marketers (click for more information) module. Sitecore has scheduled support for this module on Sitecore CMS 6 to be phased out by March 31st, 2010 and will only provide limited support until this date.  All new projects should utilize the new Web Forms for Marketers going forward, and it is encouraged that existing projects start to transition to this new forms technology over the next 6 months.
