Sitecore V5 users can choose whether existing files should be overwritten during uploading. This parameter is set on one of the steps in Upload Wizard. If user leaves this option unchecked and appropriate file already exists, new file will be uploaded with a number added to the filename (e.g. ExistingName.jpg  » ExistingName001.jpg). You can change this behaviour by implementing a custom upload process.
